Both stimulate your own collagen. Radiesse adds visible structure at the visit and keeps building afterward; Sculptra works gradually across a short series of sessions. Which one fits depends on the area, how quickly you want to see change, and how many visits you are willing to make.
Most dermal fillers are hyaluronic acid gels that hold water and add volume where they sit. Sculptra and Radiesse belong to a different group, often called biostimulators: rather than simply occupying space, they prompt the skin to make more of its own collagen. Both are FDA approved. Where they part ways is how much they do on the day.
Sculptra is poly-L-lactic acid, a material medicine has used for decades in dissolving sutures. Placed deep in the skin, its particles signal the cells that make collagen to get to work, then are slowly absorbed, leaving your own new collagen behind. Very little shows at first beyond swelling that settles in a few days. The real change builds over the following weeks and keeps improving for months, which is why it is given as a series. More on how long Sculptra lasts.
Radiesse is calcium hydroxylapatite: tiny mineral microspheres carried in a smooth gel. The gel gives immediate lift and volume where it is placed. Over the next months it is absorbed while the microspheres act as a scaffold that collagen grows around, so the support stays after the gel has gone. It is a firmer, structural product, which is why it suits bony contours such as the jaw and chin. More on how long Radiesse lasts.
The same questions, answered for each product. Longevity varies from person to person, so treat these as typical patterns rather than promises.
| Question | Sculptra | Radiesse |
|---|---|---|
| What it is | Poly-L-lactic acid, a biodegradable material injected into the deeper layers of the skin. | Calcium hydroxylapatite microspheres suspended in a smooth gel. |
| When results appear | Gradually. Early fullness from swelling settles, then collagen builds over weeks and keeps improving for months. | Volume and lift show the same day, then continue to improve over the following weeks as collagen forms. |
| Typical number of visits | A short series, often two to three sessions spaced several weeks apart. | Volume shows at the first visit. The number of visits, including maintenance visits that extend the result, is set in your plan at consultation. |
| Best-suited areas | Broad volume loss: temples, cheeks, smile and marionette lines, chin and jawline, plus the decolletage. | Structure: jawline, chin, cheeks, deeper smile and marionette lines, and the backs of the hands. |
| How long it tends to last | Often two years or more. | Often around a year or more, depending on the area and your metabolism. |
| Can it be dissolved? | No. It cannot be reversed once injected. | No. Hyaluronidase, the enzyme that dissolves hyaluronic acid filler, does not dissolve it. |
| Downtime | Minimal. Mild swelling, redness or tenderness for a few days; you may be asked to massage the area. | Little to none. Mild swelling, redness or bruising that resolves quickly. |
Neither is the better product overall. Each is the better tool for some faces, some areas and some timelines, and some faces suit neither. These are the common situations.
Volume loss across the whole face. When the temples, cheeks and lower face have all thinned a little, Sculptra tends to suit: it rebuilds collagen over a wide area, and the change accumulates over months rather than appearing on a single day.
One area that needs structure now. A flattened cheek or a softening chin before an event points toward Radiesse, because its gel gives visible lift at the visit and the collagen it prompts continues the work afterward.
Jawline or hands. Radiesse. Our practice uses it often for both, defining the line of the jaw and restoring volume to the backs of the hands.
Your first filler. Neither can be dissolved, so a first-time patient who wants the option to undo a result may be better served by a hyaluronic acid filler first. If collagen is the goal, Sculptra's slow arc lets you watch the change and decide as you go.
Hollow under the eyes. Radiesse is too firm for the delicate under-eye skin. Sculptra can help some patients by rebuilding support in the upper cheek beneath it, which our article on Sculptra for under-eye hollowing explains.
The reasons to wait or choose differently are much the same for both products. None of these is a diagnosis; each is something to raise at your consultation so your provider can decide what is safe for you.
Pregnancy or breastfeeding. Cosmetic injectables are generally postponed until after pregnancy and breastfeeding.
Infection or inflammation at the site. An active breakout, cold sore, rash or other skin infection where the product would go means treatment waits until it has cleared.
Allergies. A history of severe allergies, or a known reaction to an ingredient in either product, is something your provider needs to hear about before anything is planned.
Bleeding problems and blood thinners. A bleeding disorder, or medication that thins the blood, changes what is safe and how bruising is managed.
Expectations the product cannot meet. Neither replaces surgery for heavy sagging skin. If your goal is lifting loose skin rather than restoring lost volume, we will say so and suggest a better place to start.
You can request your consultation for Sculptra or Radiesse with one of our nurse practitioners, Lauren Barnhill, MSN, RN, FNP-C, or Sarah Lockwood, MSN, RN, AGACNP-BC, by choosing your preferred provider when you book. Because the two products work so differently, the useful part of that visit is deciding which one belongs where on your face, including when the answer is neither.
They work within the practice Dr. Shalini Gupta, MD founded in Cincinnati in 2001, with the same approach to injectables the practice has always taken: proportion first, and a result that still looks like you.
Some treatment plans use more than one product, each placed where it suits best, for example structure along the jaw and gradual volume higher in the face. Whether that makes sense for you, and in what order, is decided at your consultation.
Usually Sculptra, whose results can reach two years or more; a Radiesse result more often holds for around a year. Maintenance visits extend either one.
Discomfort is minimal for most patients. Numbing cream is available before either treatment, and a visit takes 30 to 60 minutes for Sculptra and 30 to 45 minutes for Radiesse.
Both look natural when placed well. Sculptra's change is slow enough that people see you as rested rather than treated, and Radiesse's lift on the day is kept modest, with collagen adding to it over the following months.
Usually not. The under-eye area calls for a softer product, and a hyaluronic acid filler such as Juvederm is generally the better match there than Radiesse.
Your provider looks at where your face has lost volume or structure, asks about your health and medications, and explains which product suits each area and why. You leave with a plan, the number of visits it involves, and a quote, since both are priced by the amount of product your plan needs.
If you already know which product interests you, the Sculptra treatment page and the Radiesse treatment page cover each visit in detail, from areas treated to aftercare. To see what Radiesse can do, browse the Radiesse before and after gallery.
If you are still deciding, that is what a consultation is for. Tell us where you have noticed the change and how soon you would like to see a difference, and we will recommend one, the other, or something else entirely.
Results vary from person to person. Nothing on this page is a guarantee of your own outcome, and candidacy and treatment planning are decided by a licensed provider at consultation.
Tell us what you would like to soften or restore. We will come back with which product fits, how long it lasts, and what to expect at your first visit.
