Volume Loss

Faces lose fat and bone support with age, which flattens the cheeks and drops everything below them. Correcting the volume loss at its source is usually what softens the folds it creates.

What is actually going on

The face is not padded evenly. It is built on fat pads that sit over the cheekbone and the lower face, and those pads rest on bone that also changes with age. The pads shrink and slide downward, the bone around the eye socket thins, the jaw loses a little height, and the mid-face loses the shelf it used to sit on. That is why the change reads as flattening on top and heaviness at the bottom rather than as generalized thinning.

The practical consequence is that the crease you dislike is usually driven in part by volume loss in the mid and lower face. A nasolabial fold deepens with time, weight loss, sun exposure, and the collagen loss that goes with them. A marionette line appears because the tissue beside the chin no longer has support. Treating those creases directly is treating the symptom, which is why we often inject the cheekbone when the complaint is about the mouth.

The distinction that decides everything is deflation versus laxity. Deflated tissue needs support put back under it. Loose tissue needs to be tightened or lifted. Many faces have both, so a consultation starts by finding where the support went before anyone picks a product.

Put the support back, or rebuild it

Dermal Fillers is the broad category, and within it the products behave differently enough that the choice is worth understanding. Juvéderm is hyaluronic acid, a soft water-binding gel that gives volume the same day it is placed. It moves naturally in expressive areas like the mouth and it is reversible, because hyaluronidase dissolves it if the result is not what you wanted. That reversibility is the reason it is almost always the right first filler.

Radiesse is calcium hydroxylapatite, a firmer material that holds its shape under pressure, so it does the job hyaluronic acid struggles with: keeping projection at the cheekbone and a crisp line along the jaw. It also stimulates some collagen of your own as the carrier gel is absorbed. The tradeoff is that only hyaluronic acid fillers can be dissolved with hyaluronidase, and Radiesse is too stiff for soft mobile tissue, so it belongs in structural places and not near the lips.

Sculptra is not really volume at all. It is poly-L-lactic acid, a stimulus that prompts your own collagen to rebuild over a series of sessions spaced several weeks apart. Nothing looks different the week you have it, and the improvement arrives across months. That suits diffuse, all-over deflation better than any single injection, and it suits people who would rather nobody noticed a date on which their face changed. Bellafill sits at the far end of the timeline: a semi-permanent filler, collagen gel carrying PMMA microspheres that the body usually does not absorb, so they stay in place as a scaffold, preceded by an allergy test about a month before treatment. It lasts years rather than months, which means the decision lasts years too.

Chin & Jawline Enhancement is a plan rather than a product. It uses firmer material along the jaw border, at the chin, and at the angle below the ear to rebuild the frame of the lower face. It is worth naming separately because a naturally soft jawline or a recessed chin is its own indication, treated at the jaw and chin rather than at the cheeks.

Where to start, and what to skip

If you have never had filler, start with hyaluronic acid in one area and stop there. Juvéderm placed in the mid-face tells you a great deal about how your tissue responds, how long product lasts in you specifically, and whether you actually like the change, and if you do not, it can be dissolved. Nobody should learn those things with a material they cannot take back.

If you already know how your face behaves, the rule is firm material in structural places and soft material in mobile ones. Radiesse or a jawline plan where you need an edge, hyaluronic acid where tissue folds and stretches every time you speak. Sculptra is the better answer when the complaint is that everything looks a little emptier rather than that one area has collapsed, and Bellafill is a reasonable choice only for someone who has already had the same correction repeatedly and is certain about it.

A fold is better approached by correcting the volume loss at its source than by filling the crease itself. We treat on what the face looks like rather than on a fixed calendar, which is how we avoid an overfilled appearance. We would rather under-correct and see you again in two weeks, once the result has settled.

What filler will not solve on its own is significant loose skin. Fillers suit small areas of laxity, but moderate to severe looseness is a conversation about tightening or lifting instead. A consultation exists mostly to sort that out: to find which compartments are empty, which tissue has simply dropped, and how little product it takes to change the impression rather than the face.
